AFFIDAVIT OF EMILY WINTER

CONCERNING LOST OR MISSING TOOTH

Affiant, Emily Winter, being first duly sworn, states as follows:

1.    Affiant is a resident of the City of Chicago, County of Cook and State of Illinois.

2.    On the Morning of November 30, 1989 at approximately 7:49 am Affiant became aware that she had lost her bottom, right front tooth.

3.    After making a diligent search for the aforementioned tooth, including a close inspection of her bowl of “Alpha Bits” cereal, Affiant was unable to locate the missing tooth.

4.    This affidavit is given for the purpose of inducing the Tooth Fairy to compensate Affiant in the usual and customary manner for the aforementioned lost or missing tooth. Affiant further states that if so compensated, she shall not seek additional compensation if the tooth is located in the future.

I, Robert M. Winter, a notary public, in and for the county of Cook, State of Illinois, DO HEREBY CERTIFY that Emily Winter personally known to me to be the same person whose name is subscribed to the foregoing Affidavit, appeared before me this day in person and acknowledged that she signed, sealed and delivered the said instrument as her free and voluntary act, for the uses and purposes therein set  forth.

GIVEN UNDER my hand and official seal: this 30th day of November, 1989.
